At Healing Fields, we envision communities where every person has access to continuous, compassionate, and quality healthcare and knowledge for management. Our Continuum of Care Model connects prevention, treatment, and recovery, ensuring no one is left behind on their health journey.

Rural families face many barriers—limited knowledge, high costs, distance, and social constraints. Community Health Entrepreneurs (CHEs) — trained women leaders from the community—bridge these gaps by bringing health services, information, and follow-up care directly to households. 

Awareness and Prevention

CHEs lead simple, engaging sessions on nutrition, hygiene, maternal and menstrual health, and non-communicable diseases, empowering families to recognize risks early and take preventive action.

Screening and Early Detection

Through home visits and village health camps organised with Government health centers, CHEs screen for conditions like diabetes, hypertension, and anemia by measuring blood pressure, blood sugar, and haemoglobin—enabling timely care before complications arise.

Access to Primary and Specialist Care

CHEs help families access doctors through teleconsultations and connect them to nearby government or private health facilities if there is a need for further management.  By doing so, they make sure that distance or cost never stands in the way of treatment.

Follow Up and Adherence

CHEs continue to support patients after consultations by explaining prescriptions, encouraging adherence to treatment, and guiding healthier lifestyle choices.

Long-term Wellness

Communities are encouraged to take ownership of their health outcome through continuous education and village-level health committees that sustain progress. Once the community knows how to harness the available resources, they are empowered to make educated decisions on their health.
