- The 4E Methodology drives our training and capacity-building programs, ensuring that empowerment remains an active, outward-facing, and continuous process.
- The Phygital Pathway defines our engagement model, integrating in-person interactions with technology to create a seamless and effective healthcare delivery system in resource-poor areas.

CONTINUUM CARE MODEL - BRIDGING HEALTHCARE gAPS
CHE as the central driving force
Access to healthcare and consistent engagement with it are critical for the well-being of families and entire villages. However, gaps persist—basic healthcare services often lack localization, breadth, and continuity, preventing both citizen beneficiaries and the State from fostering lasting attitudinal change.
Our Continuum of Care Model places the Community Health Entrepreneur (CHE) at its core, ensuring sustained healthcare delivery. This model follows a structured approach:
- Education & Training – CHEs are trained to understand healthcare needs and act as health advocates within their communities.
- Awareness & Demand Generation – They drive awareness, focusing on maternal and child health and non-communicable diseases (NCDs) to create demand for healthcare services.
- Family Counseling & Integrated Care – CHEs provide family counseling on NCDs, offering preventive advice that improves overall family well-being.
- Screening & Access to Healthcare – They mobilize health screening camps in collaboration with PHCs & CHCs and facilitate telehealth consultations, bridging the access gap.
- Technology-Enabled Solutions – HFF provides easy-to-use digital tools designed for the rural context, making healthcare more accessible and actionable.
- Follow-up & Behavior Change – CHEs ensure treatment adherence and behavioral shifts through consistent village-level follow-ups.
This end-to-end model ensures that healthcare access is not just available but sustained, leading to measurable improvements in community well-being.
4E METHODOLOGY
We have a unique 4E model. We train Community Health Entrepreneurs to:
● Equip them with knowledge and support and establish them as health leaders in their villages
● Provide tools, training, and resources to elevate them as entrepreneurs and telehealth facilitators.
● Through well-tested and robust training frameworks, we enable the CHEs to anchor long-term transformation in themselves and their communities.
This whole process is catalysed by digital tools, that allow CHEs to capture data that allows targeted, evidence-based, problem-solving and interventions.
