Access to healthcare and consistent engagement with it are critical for the well-being of families and entire villages. However, gaps persist—basic healthcare services often lack localization, breadth, and continuity, preventing both citizen beneficiaries and the State from fostering lasting attitudinal change.
Our Continuum of Care Model places the Community Health Entrepreneur (CHE) at its core, ensuring sustained healthcare delivery. This model follows a structured approach:
This end-to-end model ensures that healthcare access is not just available but sustained, leading to measurable improvements in community well-being.
We have a unique 4E model. We train Community Health Entrepreneurs to:
● Equip them with knowledge and support and establish them as health leaders in their villages
● Provide tools, training, and resources to elevate them as entrepreneurs and telehealth facilitators.
● Through well-tested and robust training frameworks, we enable the CHEs to anchor long-term transformation in themselves and their communities.
This whole process is catalysed by digital tools, that allow CHEs to capture data that allows targeted, evidence-based, problem-solving and interventions.
